If you ring the surgery number when we are closed you will be either diverted to GP Care Wakefield or to 111, the Urgent Care Team. They will assess your needs and recommend a course of action.

OUT OF HOURS GP CARE WAKEFIELD

GP CARE WAKEFIELD is an extended out of hours service owned by all Wakefield GP Practices to cover the entire population of the Wakefield district which will go live for Church street Surgery on the 19th September 2017.

It operates from 6 – 10 pm Monday to Friday, and 9 – 3pm Saturday’s, Sunday’s and Bank Holiday’s.   Telephones will be automatically diverted during operating hours.  Outside of these times please telephone 111 or ring back during normal surgery hours if your need is not urgent.

Following triage of your problem, if deemed appropriate the service will offer you a GP appointment at Trinity Medical Centre or Pontefract General Infirmary.

There needs to be shared consent in place for the service to be able to provide safe care therefore please ensure you give explicit record sharing consent when accepting an appointment with the service.  Patients who prefer not to give shared consent cannot access this service.

The service is not available for routine matters such as repeat prescriptions 

Routine care by HCA/Nurse (Please note not all services will be available initially)

In addition to the above, during 6pm & 8pm Monday to Friday, and 9am & 1pm on Saturday’s, Sunday’s and Bank Holiday’s, patients can be seen for routine annual review of Heart Disease, Diabetes, COPD, Asthma & Stroke but only if you are well controlled and your condition is stable.  Other routine nurse care includes:

Blood tests

Routine ECG (not chest pain)

Spirometry

Review of contraceptive pill

review depo injection

smears

B12 injection

removal of sutures and clips

Emergencies Outside of Usual Surgery Hours

If you ring the surgery when we are closed you will be either diverted to GP care Wakefield or to 111, the Urgent Care Team.

They will assess your needs and recommend a course of action which may include:

• Attending Trinity Medical Centre
• Telephone advice
• Home visit (at the doctor’s discretion).

Please note that all calls are recorded.

Other options available to you, if you need help outside normal surgery hours are:

• Attending the walk-in centre at King Street Health Centre, 47 King Street, Wakefield, WF1 2SY (open 7 days a week 8am - 8pm).

Accident and Emergency (A&E)

A&E departments assess and treat patients who have serious injuries or medical emergencies. Generally, you should visit A&E or call 999 for problems such as:

• Loss of consciousness
• Persistent, severe chest pain 
• Severe shortness of breath 
• Heavy bleeding 
• Head injuries 
• Suspected broken bones.

If you're injured or seriously ill, you should go, or be taken, to A&E. If an ambulance is needed you can call 999.

Call 999 in an emergency

House Visits

House visits are for patients who are too ill or infirm to travel to the surgery.  Please telephone the surgery before 11am on 01924 217 999 / 01924 217 667 to request a visit.  A doctor may ring you back at approximately 12 noon.  All visits are done between 11am and 2pm.

What is NHS 111?

NHS 111 is a new free telephone service being introduced nationally to make it easier for people to access local health and social care services when they have an urgent need. It's a general number for people to call when their need is urgent but not considered life-threatening (999).

It will be available 24 hours a day, 7 days a week and 365 (366 in a leap year) days a year. Calls from mobiles and landlines are free and the service is intended to respond to people’s health and social care needs when:

• it is not a life-threatening situation, and therefore is less urgent than a 999 call
• the patient does not have a GP to call or a GP is not available
• the caller feels they cannot wait and is simply unsure of which service they require
• the caller requires health information or reassurance about what to do next.

If you call within surgery hours and it's appropriate to be seen by your GP, you will be advised to contact the surgery.

If you need urgent attention out of hours you wll now be directed to call 111, the answerphone message has been updated to reflect this.

Practice Health Champions

• Practice Health Champions are people who voluntarily give their time to work with the staff in their local GP Practice or surgery to find new ways to improve the services that the practice offers, and to help to meet the health needs of patients and the wider community.
• Practice Health Champions build upon the success of the Community Health Champion model.  In addition to individuals using their life experience, understanding and position to influence their friends, families and work colleagues to lead healthier lives – Practice Champions also use their passion and understanding of the local community to enrich decision making in GP practices through the development of a greater understanding of local need; Champions develop new groups to support the health and wellbeing of the local community and also develop new skills and experiences which support their own personal development and growth.
• Practice Health Champions are supported to work with surgery staff as peers, to improve services and develop opportunities.
• The work of the Practice Champion varies, depending on their interests, experience, the needs of the practice and local community health priorities.
• Practice Health Champions are recruited and trained from the local community.  Champions reflect the needs and aspirations of the community that the GP Practices serve.

There's an information session at Church Street Surgery between 10am and 11am on Tuesday 28th April 2015.

The workshops / training will take place on Wednesday the 13th May and 20th May 10 - 3pm.  

Places are limited so please apply in advance by completing the application form above.  Financial support is available towards the cost of childcare and lunch will be provided during the workshops.

You will need to be able to volunteer a few hours each week to work alongside our Practice.  

                        New Hours for Walk in Centre to Meet Patient Need

From the 1^st November the Wakefield Walk in Centre on King Street will have new opening hours. The service will be available from 10am until 10pm 7 days a week.

The new hours mean the service is available later in the day until 10pm, and opens slightly later at 10am. The change reflects patient use of the service, which showed greater demand for the service in the evenings, rather than the early morning.

Providing same day support for patients, the centre can provide urgent care for an acute condition that may be dealt with in a single visit to a clinician.

The change in hours also reflects the new GP Care Wakefield extended hours scheme. GP Care Wakefield offers patients access to medical support across the district by calling their own GP number from 6pm until 10pm weekdays and 9am-3pm weekends and bank holidays.

Services offered by the Walk in Centre include:

Treating minor injuries and illnesses, including but not limited to:

Wounds, e.g. superficial cuts and bruises, minor burns and scalds, insect or animal bites, risk of tetanus, minor head injuries- (except in under 2’s and over 65’s as in NICE guidance)

Muscle and joint injury, e.g. strains and sprains, back pain, tendonitis;

High temperature

Headaches and dizziness

Coughs, colds, ‘flu-like’ symptoms and hay-fever

Ear, nose and throat infections (e.g. minor infections such as sore throats and ear aches)

Eye care e.g. conjunctivitis, styes, removal of superficial foreign bodies

Stomach ache, indigestion, constipation, vomiting and diarrhoea

Dermatology and skin complaints e.g. rashes, minor allergic reactions, scabies, head lice, sunburn

Emergency contraception

Post operation wound care

Acute psychological conditions and crisis resolution.
