Practice Health Champions are people who voluntarily give their time to work alongside staff in their local GP Practice or surgery to find new ways to improve the services that the practice offers, and to help to meet the health needs of patients and the wider community.
The Health Champions are working with the Practice on a number of projects at the moment:
- Introducing the Appointments Guide (more information on the home page)
- Setting up some social groups
- Creating support groups for some key health issues such as diabetes and cancer (to start with)
- Supporting the community to access the health and social services that they need, when they need it.
Introducing the Appointment Guide
The appointment guide is designed to help you make your 10 minute consultation as productive as possible, we hope that by doing this you and your clinician will get the best possible outcomes from the consultation.
We hope that this will help you have more control over your health and will ultimately improve your health, and the experience you have when you come to the Practice. By completing the questions it means that clinicians are able to use the time they have more appropriately improving health care outcomes for you.
These will soon be available and we would appreciate it if you could complete one prior to your appointment.
We want to set up some clubs where people can come together, learn new skills, share their skills, meet friends (old and new), and most importantly have fun. Below are some of the ideas we’ve had, let us know if you have any others, and if you’re interested in joining in by completing this form. The clubs are likely to start in September and we’ll be in touch soon.
